W. 130th Community Health Center - Neighborhood Family Practice

Neighborhood Family Practice is a federally Qualified Health Center that provides medical, behavioral health, midwifery and dental services. The purpose of this project is to renovate vacant CVS pharmacy building into medical and dental clinic with a pharmacy to provide services for the community.
