Long Term Care Options Counseling

This service provides information related to housing choices, types of assistance needed, care and help providers and paying for help. One-on-one consultations are provided to weigh through options for each unique situation, considering personal values, resources and preferences. The ADRC can:

  • Determine eligibility for public funding for long-term care
  • Help prepare Medicaid applications, for those who are eligible
  • Explain long term care program choices

Homeless Services for Seniors

This short-term program, funded with federal stimulus dollars, provides case management services for limited income Cleveland residents 60+ (or disabled 50+) at risk of becoming homeless. This service is intended for those currently homeless, at risk of eviction, living in condemned property, or temporarily living with others. Service is provided by the City of Cleveland Department of Aging which also collaborates with Fairhill Partners to assist homeless persons needing help to obtain housing.

CMHA is responsible for the management and operation of the local public housing program and administers rent subsidy programs to provide eligible low-income persons good, safe, affordable housing. CMHA maintains an accredited police department to ensure safe places to live and work, and a social services department that develops programs to enhance the quality of life of its residents.
